# How do I add medical information?

> Allergies, conditions, medications and insurance for each curler, kept in the Members Portal and asked for at checkout when a program needs it.

  **Must know**

  * Medical information lives in the Members Portal under **Medical**, per curler.
  * It follows the curler, not the club: one record is shared with every club they belong to. (Saved contacts and billing addresses are different: each club keeps its own. See [What happens to my details if I belong to two clubs?](/members/my-information/what-happens-to-my-details-if-i-belong-to-two-clubs).)
  * If a league or membership requires it, checkout asks for it — unless it was updated recently (the club sets the window, 180 days by default).

  <Step title="Open Medical">
    In the Members Portal choose **Medical** (heading **Medical Information**). Pick the curler under **Select Member**.

  <Step title="Fill in the form">
    Choose **Add Medical Info** or **Edit Medical Info**. Sections are **Allergies**, **Medical Conditions**, **Current Medications**, **Dietary Restrictions**, **Emergency Contact**, **Insurance Information** (**Insurance Provider**, **Policy Number**), any **Additional Questions** the club asks, and **Additional Notes**. Your club may hide sections it does not collect.

    Choose **Save Medical Information**. The page confirms **Medical information saved successfully**.

During checkout, a **Medical Information** step appears for each curler whose program requires it (**Step x of y**). If your record is already recent it is skipped.
