# What are medical forms?

> Configure the medical questions collected at registration and control how long answers stay valid.

  **Must know**

  * The **Members → Medical Forms** page is available when a superadmin enables **Medical Forms** for your club. With it off, registration still collects whatever was already configured and members can still view their own answers.
  * Answers are visible to anyone with *members.view* (including coach and volunteer roles), the member and their guardian. Editing the form needs *settings.edit*.
  * Medical info is requested only where a league or membership has a **Medical Information Requirement**.

The page is headed **Medical Information Settings**. Under **Standard Questions** you switch on any of Allergies, Medical Conditions, Medications, Emergency Contact, Dietary Restrictions and Insurance Information. **Medical Info Validity Period** sets **Valid for (days)** so answers are re-collected after that time.

**Custom Questions** → **Add Custom Question** adds your own, as Short Answer, Yes/No, Multiple Choice or Checkboxes.

To collect the form for a league, switch on **Medical Information Requirement** on its **Requirements** step; membership groups offer the same.
